Self & Family Managed Care Program

 

The Self/Family Managed Care program is an option offered by the PMHC Home Care Program. This is a Government funded program. You may be covered for up to 80% of your hourly bill. We can guide you through the process of receiving this funding. Book a consultation with us to learn more.

To be eligible for Home Care/SFMC Individuals must:

• Be a Manitoba resident

• Registered with Manitoba Health

• Require health services or assistance with the activities of daily living

• Require service to remain safely in their homes

• Require more assistance than is available from existing supports and community resources.

STEP 1  Prairie Mountain Health must assess you or your loved one.

​

STEP 2  They will determine how many hours you or your loved one quality for.

​

STEP 3  We will walk you through all of the paperwork that follows.

​

STEP 4  You will be covered for up to 80% of your hourly bill.
